第2条 条例第3及6条所指的通知书 版本日期 27/05/2002
（1）本条例第3（1）条所指的通知书须符合附表中表格1的格式。
（2）本条例第6（1）条所指的通知书须符合附表中表格2的格式。
（3）采用附表中表格2的通知书如签上或印有代表有关主管当局行事的公职人员的姓名，即属有效。
第3条 定额罚款的缴付 版本日期 27/05/2002
（1）任何人如接获采用附表中表格1或表格2的通知书，可在该通知书内所述的期间内，以下述方式缴付有关定额罚款─
（a）以邮递方式寄往库务署收；
（b）亲自或透过其代理人在任何邮政局（邮政局信箱或流动邮政局除外）缴付；
（c）透过银行自动柜员机；
（d）藉电话使用一般称为“缴费聆”的服务；
（e）透过互联网；或
（f）透过银行提供的电话理财服务。
（2）任何人如意欲按照第（1）（a）或（b）款缴付定额罚款，须将有关通知书连同款项按照该通知书列明的缴款办法一并交付。
（3）凡按照本条缴付任何款项，所缴付的款项须为通知书所指明的款额；如就多于1份通知书缴款，则所缴付的款项须为该等通知书所指明的款额的总和。
（4）按照第（1）款缴付的款项，不得包括并非该份或该等通知书所指明的事宜所关乎的款额，亦不得构成该款额的一部分。
（5）任何款额如非按照本规例缴付，库务署署长可将款额退回付款人。

1. 如于本通知书发出日期起计21天内缴付定额罚款，可解除就本通知书所指明的罪行所须负的法律责任。必须按照缴款办法缴付罚款。
1. Liability for the offence specified in the notice may be discharged by paying the Fixed Penalty within 21 days of the date of issue of this notice.
Payment must be made in accordance with the Payment Instructions.
2. 你若意欲就该罪行的法律责任提出争议，可以书面方式通知主管当局。请注明本通知书左上角所示的10位数字档号以作参考。
2. If you wish to dispute liability for the offence， you may inform the Authority in writing.
Please quote the 10-digit Reference No. shown at the top left hand corner of this notice for reference.
3. 如你不按照本通知书缴付罚款或通知主管当局你意欲就该罪行的法律责任提出争议，则可能须缴付附加罚款。
3. Failure to pay in accordance with this notice or to notify the Authority that you wish to dispute liability for the offence may result in an additional penalty being imposed.

Environ Hygiene Dept“ for payment.
（b） 使用「缴费聆」缴款
缴款前，请先致电18013登记本通知书。请选择“商户编号”“9147”。如欲缴款，请致电18033，并按照指示输入所需资料。如需查询，请致电「缴费聆」热线：2311 9876.
（b） By using Payment By Phone Service （PPS）Before making payment， please dial 18011 to register this notice.
Please select the “Merchant Code” of “9147”。 For making payment， please dial 18031 and follow the instructions to enter the information required.

（c） 透过互联网缴款
可透过银行及「缴费聆」在互联网上提供的缴费服务缴款。详情请浏览「公共服务电子化」计划的网站（URL位址：//www.esd.gov.hk）或库务署网站（URL位址：//www.info.gov.hk/tsy）。亦可透过「公共服务电子化」计划的服务站缴款。缴款时，请选择“食物环境衞生署”。

Payment can be made through the bill payment services provided by banks and PPS on the Internet.
For details， please visit the web site of the Electronic Service Delivery （ESD） Scheme （URL address： //www.esd.gov.hk） or the web site of Treasury （URL address： //www.info.gov.hk/tsy）。
Payment could also be made at any of the ESD kiosks. Please select “Food
Environ Hygiene Dept“ or ”Food and Environmental Hygiene Department“ as appropriate for payment.
（d） 透过电话理财服务缴款
可使用银行提供的电话理财服务，透过有关的缴费服务缴款。缴款时，请选择“食物环境衞生署”。
（d） Through Phone-banking Service Payment can be made through the bill payment services by using the phone-banking service provided by banks. Please select “Food and Environmental Hygiene Department” for payment.
（e） 邮递缴款
凡邮寄支票、汇票或本票缴款，请连同邮寄缴款回条邮寄香港告士打道邮政局信箱28000号库务署收。请勿邮寄现金。请将本通知书下半部所示的16位数字编号写在支票、汇票或本票背面。请注意：以邮递方式缴款将不会获发收据。有关的邮戳日期会被视为缴款的日期。
（e） By Post A cheque， draft or cashier order should be sent together with the Slip for Payment by Post to the Treasury， P.O. Box No. 28000， Gloucester Road Post Office， Hong Kong. Cash should not be sent through the post.
Please write the 16-digit serial number shown at the lower portion of this notice on the back of the cheque， draft or cashier order. Please note that no receipt will be issued for payment by post. The date of the relevant post mark is to be regarded as the date of payment.
（f） 亲自缴款

Payment made before mid-night on the due date will be regarded as on-time payment.
3. 支票、汇票或本票均须以“香港特别行政区政府”或“香港特区政府”为收款人，并加以划线。以支票付款，须在支票首次提交付款银行获得兑现后，缴款方为有效。期票恕不接受。
3. Cheques， drafts or cashier orders should be made payable to “The Government of the Hong Kong Special Administrative Region” or “The Government of the HKSAR” and crossed.
For payment made by cheque， payment is valid only when the cheque is honoured on the first presentation to the drawee bank. Post-dated cheques will not be accepted.

If you do not pay the Fixed Penalty or notify me that you wish to dispute liability in accordance with this notice， an application will be made to a magistrate for an order that you pay the Fixed Penalty， an additional penalty equal to the amount of the Fixed Penalty and the sum of $300 by way of costs.

欲就法律责任提出争议者请注意 NOTES FOR THOSE WHO WISH TO DISPUTE LIABILITY
1. 若你意欲就上述罪行的法律责任提出争议，可签署附上的表格，并于通知争议的最后日期或该日之前将之交付主管当局，以告知主管当局你有此意欲。
1. You may notify the Authority， if you wish to dispute liability for the offence， by signing the attached form and delivering it to the Authority on or before the last day for notification of dispute.
2. 如你告知主管当局欲就上述罪行的法律责任提出争议，事件会以申诉方式交由裁判官按照本条例裁定，传票将会在适当时候向你送达。
2. If you notify the Authority that you wish to dispute liability for the offence， the matter will be determined by a magistrate on complaint in accordance with the Ordinance and you will be served with a summons in due course.
3. 如你在告知主管当局欲就上述罪行的法律责任提出争议后，在裁判官席前应讯时你并没有提出免责辩护或你提出琐屑无聊或无理取闹的免责辩护，则在任何其他罚则及讼费以外，可被判缴付相等于定额罚款数额的附加罚款。
3. If， having notified the Authority that you wish to dispute liability for the offence， you appear before a magistrate and offer no defence or a defence which is frivolous or vexatious， you are liable to， in addition to any other penalty and costs， an additional penalty equal to the amount of the Fixed Penalty.
4. 如不按照本通知书缴付罚款或通知主管当局你意欲就法律责任提出争议，则可能须缴付附加罚款。
4. Failure to pay or notify the Authority that you wish to dispute liability in accordance with this notice may result in additional penalty being imposed.
